An image is counted as missing alt when the <img> tag has no alt attribute at all. Empty alt (alt="") is reported separately because it is the correct choice for purely decorative images, but wrong for informative ones.
The tool reads the initial server-rendered HTML. Images injected later by JavaScript (some React/Vue components, lazy carousels, third-party app widgets) are not included. Most Shopify themes ship images in the initial HTML, so the count is usually accurate for storefronts.
